What to Bring With You
- At least 2 originals Certificates of Origin, one for you and one for the Chamber. (The Chamber does not supply the Certificate of Origin)
- Certificate of Origin
- Must state the Country of Origin as "USA" or "United States of America"
- Must include the Chamber's wording (see Helpful Tips below) at the BOTTOM OF THE PAGE below the exporter's declaration
- Must be signed by exporter PRIOR TO being signed and stamped by a Chamber representative
- Backup documentation to prove products originated in the United States
- This includes, but is not limited to, Bill of Lading (or other shipping documentation), Commercial Invoice, or Packing List
- Must match-up with the Certficiate of Origin
- We cannot issue a stamp if any products or materials are listed as originating outside of the United States
- Certificate of Origin
- Payment in the form of Credit Card, Check, or Exact Cash
- Contact the Chamber for the price of the Certificate or Notary service

- Payment is due at the time of signing
- We CANNOT stamp commercial invoices
- Make sure to fill out your Certificate before coming in

The Southwest King County Chamber of Commerce, a recognized Chamber of Commerce under the laws of the State of Washington, has examined the manufacturer’s invoice or shipper’s affidavit concerning the origin of the merchandise and, according to the best of its knowledge and belief, finds that the products named in this certificate originated in the United States of America. This certification does not certify origin for purposes of the laws of the United States of America and makes no warranty, express or implied concerning the goods, affidavit, or documents relating thereto, and assumes no responsibility for the truth or accuracy of any statements contained in said affidavit of any of the documentation mentioned therein.
